I travel to the US regularly and, as an EU citizen, have recieved a stamp every time. However, re-entering the US from Canada, I did not recieve a stamp because the green visa thing they staple in my passport was not removed at Canadian customs
Passport was stamped leaving Toronto for Miami. Green card (I94) wasn't collected when I left Miami for El Salvador. When I returned from Panama passport was stamped again and another green card issued. Immigration told me to make sure both were collected at boarding gate when I left country.
I also have used this to my advantage. When on a plane back from the US, I realised that my green I-94W thing was still in my passport. Fearing repercusions upon my return to the Land of the Free, I went up to Mr LHR Immigration, explained the situation, and he kindly gave me a Heathrow stamp -- although he ruined it slightly by marking it 'by request'
One thing to watch out for, if you are entering the USA by land (from Canada or Mexico), and you have surrendered your green bit of paper, is that they will charge you a fee ($4 or something) to enter the USA again -- this is included in flight/ship fares, but if you're stuck on the border without dollars, that could be a problem...
Pre-my current passport (issued 2002), I made two trips outside the US (I'm a US citizen), both times coming through JFK. Only on my first visit did they stamp my passport upon entry.
On my current passport, I've been out of the country about six times - granted, most of those times were on cruises. But when I took the ferry to the Bahamas (and stayed three days), flew to Ireland, flew to the Philippines, and flew to Turks and Caicos, it was only when I landed at JFK was my passport stamped - twice - unasked. FLL (Ft. Lauderdale) and CLT (Charlotte-Douglas) did not.
